Bactrim is listed as FDA Pregnancy Category C. Women who are pregnant should be extremely cautious when using Bactrim and should only use it if the benefits truly outweigh the risks. They should make this decision with the help of their doctor. There is little conclusive evidence about the possible adverse effects on a breastfeeding infant. Therefore extreme caution is also advised for breastfeeding mothers when considering the use of Bactrim to treat an infection.

People taking Bactrim should not drink alcohol during treatment. This includes not only alcoholic beverages, but also cough medicines and mouthwash containing alcohol. Alcohol can intensify the negative side effects of Bactrim to even dangerous proportions. These possible side effects include:
- Headache
- Nausea or vomiting
- Rapid heart rate (tachycardia)
- Shortness of breath
Nausea and vomiting can cause dehydration and vomiting up the medicine results in a missed dose. However, alcohol does not reduce the effectiveness of Bactrim, so if you drank alcohol during treatment, you do not need to worry that the treatment was compromised or made ineffective, unless you vomited a dose.

Although Bactrim has not been shown to have adverse reactions in children, caution is still advised. The younger the child, the more cautious you should be about giving him or her Bactrim. Bactrim should not be administered to young infants (especially infants aged two months or under) as it has not been tested on them and its possible effects are still unknown.
